Athlete Waiver for kids sessions
I understand I/my child is participating in strength and conditioning sessions offered by Project YOU Coaching.
I understand that it is my responsibility as the parent to consult a medical professional prior to and regarding my child’s, participation in any session. I respect and warrant that my child is physically fit and has no medical condition that would prevent their participation in the session.
In consideration of being permitted to participate in any group fitness session, I agree to assume full responsibility for any risks, injuries, or damages to my child, known or unknown which might incur as a result of participating in the session.
In consideration of being permitted to participate in any session, I knowingly, voluntarily, and expressly waive any claim I may have against Project YOU Coaching for injury or damages that I may sustain as a result of participating in the session.
I give my permission to Project YOU Coaching to procure medical attention for my child in the event of an injury, accident, and/or illness to my child while participating in PYC S&C classes which, in the opinion of a paramedic or a medically trained practitioner, requires urgent medical attention and a parent, legal guardian or any given emergency contact are not available.
